What is the Commonwealth Home Support Program?

The Commonwealth Home Support Program (CHSP) is a Government-subsidised program for over 65s who need a little bit of extra support to stay at home. The program provides subsidised home and community care services which are delivered by service providers like Vision Australia.

With CHSP, the focus is on working with you, it’s about building on what you can do, to help you stay in your home - independent, happy, healthy and safe.

The CHSP is designed to deliver low-level support and social connections. Vision Australia can help you apply for the CHSP to remain independent at home.

As your needs become more complex or intensive and you start to require case management assistance, you may be referred for a Home Care Package (HCP).

Vision Australia can help you apply for an HCP and coordinate and manage the home and community services that meet your needs on your behalf (check our locations first).

Vision Australia CHSP services

The CHSP offers a range of home and community care services. Vision Australia is a specialised provider who can support you with a wide range of wellbeing and restorative CHSP services specifically tailored to support people with vision loss.

We offer extensive allied health services, specialised support programs, aids and technology support, through to extensive social programs. We can support you to be active in your community and join our community for health, wellbeing and social programs. Our CHSP services provide all the support you need to have more good days at home.

Who is eligible?

You may be eligible for Commonwealth Home Support Program (CHSP) funding if you are:

  • having trouble doing everyday activities without help, and
  • need support to live independently in the community

and are:

  • Aged 65 years or older
  • Aged 50 years or older and identify as an Aboriginal or Torres Strait Islander person
  • Aged 50 years or older (45 years or older for Aboriginal and Torres Strait Islander people) and on a low income, homeless or at risk of being homeless.
